This is completely up to you and not a requirement. SOS will discuss possible options with you when we connect with rea Mom in need in your area.

I agree to keep Mom's information and situation confidential and keep her and all details of her situation private. SOS gathers detailed personal information regarding Mom's situation and in order for Mom to feel comfortable to disclose personal details, her trust is the only way we can truly be sure to provide the support she needs.

I agree to commit to a minimum of one (1) full year as a Sister on the Ground to the Mom/family I am responding to helping - or as long as they stay in contact.

SOS decisions re: Mom *

I agree to abide by any decisions made by the SOS Board as to ongoing contact with the Mom. There may occasionally be decisions to cease contact if it is determined that Mom is not acting in the best interests of herself and/or her child(ren).

Keep SOS updated *

I agree to keep SOS updated with details of meetings, phone calls or as new situations/needs arise where Mom needs additional assistance. (For example advise SOS if a Mom needs additional supplies or is not taking steps to establish independence and/or making forward progress or acting on recommendations provided to her.)

Keep SOS UTD w/services *

I agree to keep SOS informed of all support of any kind that I or my personal network provide to Mom to ensure that Mom is receiving support to ensure long term success and so that supplies and services are not duplicated .

I understand that being a Sister On The Ground does not require me to donate monetarily to the Mom/family or to SOS.

ROI Consent *

Consent for Release of Information: I understand that the Saving Our Sisters (SOS) Board of Directors will have access to this information for the purposes of overall program evaluation and program improvement. SOS will never share my name, identifying information or image without my written authorization with the public.
